Health Statement
to be completed and signed by your child's physician and returned along with a current copy of your child's immunization records by August 3rd (required)
Please note that children will not be allowed to attend without an updated Health Statement and Immunization Records on file

Food Allergy Action Plan
required if your child has a food allergy and returned by August 3rd with the physician's signature

Parent Guide
by the Temple Emanu-El Early Childhood Education Center
you will be asked to sign an authorization that you have received this guide
